Setting Up Your Patient Portal
Step 1: Click Patient Portal
Step 2: Click "Setup Account"on the Login Portal screen of the patient hub
Step 3: Enter the email address that you use with PATH and click Submit.
Step 4: Find the login code that has now gone to your email.
Step 5: Enter this code in the Login Portal screen and create a password for your account.
Your default username should be the first part of your email address before the "@" sign. If this doesn't work, try using the whole email address.
If you use the same email address for multiple patient profiles, you will need to designate different usernames
Passwords are case-sensitive
FOR FURTHER HELP with the portal, PLEASE EMAIL firstname.lastname@example.org with what you would like your username and password to be and it can be manually reset along with a confirmation email for your records.
You will now have access as a patient to the PATH Integrative Health Patient Portal where you may access your labs, records, and other documents, as well as submit supplement requests, and view your balance.
